To enroll for an ALS 5-Day Hearing Healthcare “New Dispenser” Training Class please complete the application form below. When submitted, an ALS 5-Day Hearing Healthcare “New Dispenser” Training Class representative will begin processing your student enrollment application. An ALS 5-Day Hearing Healthcare “New Dispenser” Training Class representative will contact you via telephone or email to confirm your enrollment. Class space is limited to the first 10 students per class.

Total Tuition: $3,695.00 (All Inclusive)
Minimum deposit required to enroll: $500.00
Tuition Balance: $3,195.00 (due 30 days prior to class date)

Tuition includes hotel accommodations in Daytona Beach, Florida for five (5) nights, most meals, airport shuttle service, daily transportation and much more.

Applicants making payment by credit card hereby authorize Alan Lowell Seminars, Inc. to charge the applicant’s credit card for the additional unpaid tuition balance due on or before 30 days prior to the applicants scheduled class date unless other arrangements are made. Cancellation The person who completed the registration form agrees that if he or she cancels the registration within 30 days prior to the beginning of class, at the discretion of Alan Lowell Seminars, Inc. a portion up to 50% of the tuition may not be refundable if the class is booked to capacity and your reservation cannot be filled by another student. Dispute Resolution In the event of a dispute, the non-prevailing party will pay the other’s costs and attorney’s fee.
